This section provides templates for use in opting out or the data removal process.  We have experimented over the years and have found the format of these removal requests to be the most effective while providing the least amount of information to the offending site.  

The templates are meant to be a starting point and should be modified for your data removal situation.

Basic Opt-Out Form

This is a general use form for opt-out requests that can be used for both mail, e-mail, and fax.

Basic Opt-Out Form – Proof of Identity

E-Mail (First Attempt)

Use this form to e-mail the address provided on the opt-out or privacy page.  Be sure to modify to meet the site’s requirements for data removal.

Use this form to contact site administrators, customer support, or privacy officers when previous attempts have failed or not all data has been deleted.

Use this form to contact the company hosting the website to forward your request to the site sharing your information.

Law Enforcement

Law Enforcement Opt-Out Form

This is a form specifically for law enforcement for opt-out requests that can be used for both mail, e-mail, and fax.  Only use this form if you are in law enforcement.  Impersonating an office is a serious crime.
